Pharmacist Services
The SCFHT Pharmacist provides education and support relating to the use of medication.
What can we help with?
- Ensure that a patient’s medications are safe, effective and working well
- Prevent and resolve medication- related issues in conjunction with the patient’s primary care provider
- Connect with patients after a hospital stay to review medication changes
Services and Supports:
- Answer medication related questions and concerns
- Conduct comprehensive medication reviews: provide information about the purpose of each medication, side effects to watch out for, and manage potential drug interactions, simplify daily medication schedules, discuss opportunities to “de-prescribe” or stop medications which may no longer be needed
Who is it for?
- For patients with or without a primary care provider
- Patients may self-refer, or be referred
Patients may be referred to see our pharmacist, or may self-refer; you must have a family doctor.
