The Sunset Country Family Health Team Nurse Practitioner Clinic is an innovative solution to improve access to primary care services in the Kenora and surrounding area.
What is a Nurse Practitioner?
Nurse Practitioners are Registered Nurses with post-graduate education that enables them to practice independently as well as in collaboration with other healthcare providers, which includes referrals to specialists if necessary. Nurse Practitioners are fully qualified to provide diagnosis and treatment of illnesses and injuries, order and interpret medical testing, prescribe medications, and partner with you to develop health and wellness goals and personalized care plans.
Nurse Practitioners have a holistic approach to providing care, which means that they assess and help you understand how your unique life circumstances impact your health.
You must be registered with Health Care Connect
For a fair process of choosing patients, we will be selecting those who are registered with Health Care Connect and have given consent to be contacted by the Clinic. Please register with them online or by calling 1-800-445-1822 Monday to Friday, 8 a.m. to 4 p.m. CST. This is the only way to become a patient of our Nurse Practitioner Clinic.
Please do not wait until clinic opening to register with Health Care Connect. Get yourself and your family members signed up today.
