The SCFHT Memory Clinic Program provides optimized access, diagnosis and care for patients and their family navigating memory concerns.
What can we help with?
- Diagnosis and treatment for memory difficulties
- Support and education for patients and caregivers
- Comprehensive care for conditions involving memory loss
Services & Supports:
- Patient care is supported by a team of specially trained health care professionals. Health care team includes: doctors, nurses, pharmacists, community health workers, occupational therapists and community agencies
- Provide medication reviews and education for patients and their families
- Functional and cognitive impairment assessment
Who is it for?
- Patients must have a primary care provider
- Patients must be referred
